Terminating the Physician-Patient relationship
Once a Physician-Patient relationship is established, the treating Physician is required to render care so long as the patient requires attention, or until the relationship is terminated by consent of the parties or is revoked by the will of either party.
In the event that a Physician needs to terminate the relationship, it is best to send the patient a written notice of the termination by certified mail, “return receipt requested,” and by regular mail simultaneously.

Authorization to Release Confidential Health Records
Virginia law as it relates to health records privacy is located in the Code of Virginia under the citation Va. Code Ann. §32.1-127.1:03. The statute provides a form containing information which shall be included in the written authorization.
